Duke (2002) rates juniper as + (use with caution), noting diuretic, carminative, and aquaretic activities, primarily at the experimental level (score 1). Traditional use for urinary infections is not supported by strong clinical evidence. The essential oil contains potentially nephrotoxic terpinen-4-ol, and juniper is contraindicated in kidney disease, urinary infections with inflammation, and pregnancy (emmenagogue and potential abortifacient properties). Commission E and PhEur do not recommend medicinal doses for prolonged use exceeding 4–6 weeks (Duke, 2002).

Botanical Description

Evergreen coniferous shrub or small tree, often low and sprawling, with sharp, needle-like leaves in whorls of three, each bearing a single white band on the upper surface. The plant is dioecious; female plants bear small, round, fleshy, berry-like cones ('juniper berries') that take two to three years to ripen from green to blue-black.[1]

Habitat

Grows on heath, moorland, rocky hillsides and open coniferous woodland on poor, often calcareous soils; one of the most widely distributed conifers, native across the Northern Hemisphere from Europe and Asia to North America.

Native to the central Mediterranean (Sardinia and southern Italy), long cultivated worldwide as a culinary and medicinal herb in gardens and as a field crop.[11]

Harvesting

The ripe, blue-black berry-like cones are hand-picked in autumn, typically in their second or third year of ripening (a mix of ages is often present on the same shrub); leaves and young stems can be gathered through the growing season.

Leaves are picked fresh throughout the growing season; the root (of root-parsley varieties) is lifted in autumn; the seed (fruit) is collected from second-year plants once ripe and dry - it is used mainly in medicinal/aromatic preparations rather than food, given its higher concentration of the toxic essential-oil constituents apiole and myristicin.[11]

Traditional Uses

Juniper berry has a long European tradition as a diuretic and urinary antiseptic for minor urinary complaints, a warming digestive carminative for indigestion and bloating, and a flavouring for gin and other spirits; the leaf and wood have also been burned or used topically for their antimicrobial properties.[1]

Parsley has a long Mediterranean culinary-medicinal tradition as a carminative digestive tonic, breath freshener and mild diuretic traditionally used for urinary complaints and fluid retention. Modern research supports antioxidant, hepatoprotective, nephroprotective and diuretic activity.[11, 12]

Preparations

Infusion (berries)[16]

Comminuted juniper cone berries infused in 150 ml boiling water, 2.0-2.5 g as a single dose 1-3 times daily per the current revision of the EU herbal monograph. Educational reference only, not a prescription.

Steam-distilled from the berries; used in small internal doses for short periods or diluted for topical/aromatic use.

Use is limited to short courses - if symptoms persist beyond 2 weeks a healthcare practitioner should be consulted - with adequate fluid intake; contraindicated in severe renal disease and not recommended in pregnancy. Confused Part: Berry-like cones ('juniper berries') and foliage harvested for flavouring or tea; the toxic ornamental savin juniper is easily taken for edible common juniper, especially in gardens and parks.
Confusion Context: Common juniper (Juniperus communis) berries flavour food and gin, but not all junipers are edible. Savin juniper (Juniperus sabina) is a low, spreading ornamental juniper that is genuinely poisonous: a peer-reviewed analysis (Zheljazkov et al., 2021) found the antimitotic cell poison podophyllotoxin in savin but NOT in common juniper, and savin also contains irritant terpenes (sabinene, sabinol) and has long been a dangerous abortifacient. Because savin is widely planted in yards, parks and landscaping, 'juniper berries' picked from cultivated shrubs can be this toxic species; foraging guidance stresses positively identifying the species before eating any juniper.
Distinguishing Features: Foliage (decisive): edible common juniper has sharp-pointed NEEDLE-like leaves in whorls of three, each with a single white band on the upper surface. Savin and most ornamental junipers have soft, SCALE-like leaves pressed against the twig. Whorls of three sharp needles = common juniper; scale-like foliage = a juniper to avoid., Smell: savin foliage has an acrid, unpleasant (foetid) smell when crushed, unlike the clean gin-like aroma of common juniper., Habit: common juniper is a bushy shrub or small tree of wild heath and hillside; savin is a low, wide-spreading ornamental grown in gardens and municipal planting.

Confused Part: Leaves and stems (foliage); the divided leaves resemble flat-leaf parsley, especially before flowering.
Confusion Context: Poison hemlock is in the same family as parsley and its foliage is often described as parsley- or carrot-top-like; it can be confused with edible parsley-family plants. Its alkaloid coniine causes a paralysis that stops breathing, and eating even a small amount of any part can be fatal.
Distinguishing Features: Poison hemlock stems are smooth, hairless and marked with purple or red spots and streaks; garden parsley stems have no purple blotching., Crushed poison hemlock smells foul and musty ('mousy'); parsley smells clean and pleasantly herby., Poison hemlock grows tall (often 1.5 to 3 m) with hollow stems; culinary parsley is a low herb.

Confused Part: Leaves and foliage; the finely divided glossy leaves closely resemble flat-leaf (Italian) parsley — the origin of the name 'fool's parsley'.
Confusion Context: Named directly for the confusion with parsley, fool's parsley contains toxic polyacetylenes; poisoning causes burning in the mouth and throat and gastrointestinal and neurological effects, and it is especially a risk to children.
Distinguishing Features: In flower, each little umbel of fool's parsley has three or four long, narrow bracteoles hanging downward beneath it (a 'beard'); true parsley has no such long drooping bracteoles., Crushed fool's parsley smells foul, acrid and mousy; parsley smells clean and pleasantly herby., Fool's parsley leaves are hairless, more finely divided, and darker and glossier than culinary parsley.
Key Test: Look under the flower clusters for three or four long, downward-hanging bracteoles (a 'beard') — present in fool's parsley, absent in true parsley — and cross-check the foul mousy smell versus parsley's pleasant scent. If a parsley-like plant only smells 'off' and isn't flowering, don't use it without expert confirmation.
